Multiple site refers to laboratory sites operated by the same corporation/entity and meeting the following criteria, without exception:

1. All sites have the same Medical Director and Technical Director.

2. All physicians interpreting any vascular examinations at any of the sites are included in the application for accreditation in the Organization section.

3. All technologists performing any vascular testing at any of the sites are included in the application for accreditation in the Organization section.

4. All physicians and technologists from all sites participate together in quality assurance and education programs, including in-house conferences.

5. The Technical Director rotates to all sites a minimum of two full days per month for supervision of the technical staff; OR every technical staff member from each remote site works at the primary site two full days per month; OR an appropriately credentialed lead technologist is appointed at each remote site to report to the Technical Director.

6. The sites all have identical protocols.

7. The sites all have the same diagnostic criteria.

8. The sites all have the same level of equipment.


ADDING A MULTIPLE SITE OR A MOBILE SERVICE

Laboratories wanting to add a multiple site and/or mobile service to an application that has already been granted accreditation must submit the completed multiple site/mobile service supplement, an updated equipment table (if applicable), additional case studies as required by the application instructions, and the additional site/mobile component fees. Accreditation for the added multiple site or mobile service will expire when the laboratory's original accreditation expires.

THINGS TO KNOW IF YOU ARE A MULTIPLE SITE LABORATORY

If eligible, the multiple sites may apply with a single application and will be assessed an additional $750 per site (over one).

At least two additional case studies (one normal, one abnormal) per area of testing must be submitted for each additional site. The selected cases should represent as many staff members as possible.
The accreditation decision will be made for each site individually.
Multiple site laboratories that are granted accreditation will receive certificates for each testing area and each site.
All ICAVL correspondence will go through the laboratory listed on the accreditation agreement.
